Since 1985, The Brook Hospital has been offering hope to individuals, families, and communities. Located in Louisville, KY, The Brook – Dupont is an 88-bed hospital providing a full continuum of inpatient, extended care, partial hospitalization, and outpatient services for adults.

Job Duties/Responsibilities

• Facilitate inpatient discharges to the next level of care with a focus on transitioning care from the inpatient setting to The Brook Hospitals outpatient programs.
• Provide groups and individual education to patients on the importance of continuing care.
• Communicate eligibility to key members of senior leadership and associated departments and collect and enter data into Midas for trending report purposes.
• Understand and implement components of the Step-Down playbook as needed.
• Have a thorough understanding of the inpatient workflow process, particularly in case management and discharge planning.
• Other job functions may include: obtaining psycho-social assessments, conducting family/group therapy on assigned cases, completing documentation per hospital policy and procedure, case coordination, coordinating discharge planning, assisting in developing and maintaining community resource information, performing marketing functions as assigned, ensuring good clinical judgment to ensure optimum patient care, participating in continuing education activities.

Qualifications

• Education Required: Master's Degree from an accredited school in the Behavioral Sciences
• Licensure/ Certification: Must be licensed or under supervision in state of Kentucky (CSW/LCSW, CADC/LCADC, MFTA, LMFT, LPCA, LPCC, LPA, LPP)
• Experience Preferred: One to three years experience
