About the Role

The Outpatient RN CAP 2 is a professional registered nurse position within the Clinical Advancement Program (CAP). This role is accountable for providing and coordinating clinically competent care, including assessment, planning, intervention, and evaluation for an assigned group of patients. As part of the End Stage Kidney Disease (ESKD) healthcare team, this position involves decision-making, teaching, leadership, and quality improvement activities to enhance patient care outcomes and facility operations.

Responsibilities

  • Perform all essential functions under the direction of the Supervisor, Clinical Educator, Preceptor, or in collaboration with other Registered Nurses.
  • Conduct ongoing, systematic collection and analysis of patient data pre-, during, and post-hemodialysis treatment; document in patient medical records and adjust treatment plans as needed.
  • Assess, collaborate, and document patient/family’s basic learning needs to provide initial and ongoing education.
  • Direct and provide, in collaboration with direct and ancillary patient care staff, all aspects of safe and effective hemodialysis therapy delivery.
  • Administer medications as prescribed or according to approved algorithms, documenting medical justification and effectiveness.
  • Initiate or assist with emergency response measures.
  • Serve as a resource, leader, coach, mentor, and role model for new and incumbent employees.
  • Ensure correct laboratory collection, processing, and shipping procedures; reschedule missed or insufficient collections.
  • Identify expected outcomes, document, and update nursing assessments and care plans through collaboration with the Interdisciplinary Team.
  • Ensure patient awareness of transplant and treatment modality options.
  • Participate in education and quality improvement projects at the facility and area level.
  • May serve as a Preceptor to new employees and complete CAP requirements to maintain or advance.

Requirements

  • Graduate of an accredited School of Nursing.
  • Current appropriate state licensure.
  • Current or successful completion of CPR BLS Certification.
  • Must meet the practice requirements in the state of employment.
  • Minimum of 2 years of Nephrology Nursing experience (entry level for RNs).
  • Successful completion of all Fresenius Kidney Care education and training requirements for new employees (if assigned oversight of Patient Care Technicians/LVN/LPNs/RNs).
  • Minimum of 3 months experience in chronic/acute hemodialysis as an RN (if assigned oversight of Patient Care Technicians/LVN/LPNs/RNs).

Skills

  • Ability to provide direct patient care, including heavy lifting, moving patients, and assisting with ambulation.
  • Frequent, prolonged periods of standing and bending.
  • Ability to move machines and equipment of up to 200 lbs. with assistance and lift chemical/water solutions of up to 30 lbs. as high as 5 feet.
  • Ability to work in a healthcare environment with moderate noise levels and potential exposure to infectious/contagious diseases.
  • Proficiency in desk work, computer tasks, and interaction with patients, staff, and physicians.
  • Willingness to travel to training sites or other facilities as required.

Pay

Hourly Rate: $34 - $54, depending on work location, qualifications, education, work experience, skills, and competencies.
