Outpatient Physical Therapist PRN - Float Pool

Primary City/State – Outpatient Float Pool Position. Open to Ortho, Neuro, Pelvic Floor, and other Specialties. Must be available to work 12 shifts/quarter on a PRN, Day Shift schedule (8:00 am - 5:00 pm).

Responsibilities

• Implement an individualized treatment plan using skilled, evidence-based therapy techniques, including therapeutic exercises, manual techniques, functional mobility training, activities of daily living training, and modalities as indicated.
• Provide verbal and written instructions to patients, caregivers, and family members.
• Maintain current knowledge of physical therapy techniques, DNV and Medicare requirements through continuing education courses and professional journals.
• Evaluate patients per Department Operating Policy, demonstrating sound clinical reasoning in synthesizing data gathered and identifying treatment diagnoses.
• Establish an appropriate, comprehensive plan of treatment with measurable, obtainable, and functional goals, including a defined discharge plan.
• Perform ongoing evaluation of patient’s response to treatment to ensure effective and appropriate provision of services, documented thoroughly.
• Maintain accurate and current documentation in the patient’s medical record, including evaluations, daily treatment notes, updated treatment plans, progress reports, discharge summaries, and daily patient charges.
• Complete billing and billing sheets according to department procedures and within established timeframes.
• Assume responsibility for achieving and maintaining financial stewardship by utilizing all resources effectively and efficiently.
• Confer with the interdisciplinary team to clarify treatment plans and discuss patient status when indicated.
• Consult with appropriate medical personnel regarding patient needs, status, and discharge plans.
• Provide timely, proactive feedback to case managers and other key personnel regarding discharge needs, including recommended discharge level of care and equipment needs.
• Participate in interdisciplinary rounds as indicated.
• Communicate information and address concerns to achieve optimal treatment outcomes through feedback and documentation.
• Utilize effective communication and interpersonal skills to enhance and encourage collaborative staff relationships.
• Delegate appropriate patients to physical therapist assistants, supervise care delivered by therapist assistants and assistive personnel, and provide updated plans of treatment and instruction.
• Act as a resource for therapy-related issues to therapy assistive personnel and staff from other departments.
• Assist in identifying department needs and process improvements.
• Contribute to the recruitment and retention of talented staff through positive role-modeling and fostering peer relationships.
• Participate in departmental and interdepartmental staff meetings, in-services, utilization review programs, committees, and student preceptor and/or mentor programs.

Requirements

• Bachelor’s in Physical Therapy from an accredited university approved by the APTA (required prior to starting employment).
• Master’s or Doctorate in Physical Therapy from an accredited university approved by the APTA (preferred prior to starting employment).
• Six (6) months of recent experience in a network float position (preferred).
• Basic Life Support (BLS) certification (required).
• Current license issued by the Arizona Board of Physical Therapy in good standing, and/or Compact State Licensure (required upon hire).
• American Physical Therapy Association (APTA) certificate (preferred).
