Qualifications

• Active NY State license and registration as a Physician Assistant or Nurse Practitioner

• For Physician Assistants: bachelor’s degree, accredited training, and at least one year of ambulatory care experience
• For Nurse Practitioners: graduation from an accepted program, pharmacology coursework, and one year of ambulatory experience (preferred)
