36 Patient Contact Hours per week with 4 hours of admin time

The Team
Team will be located at new primary care construction site: Warsaw Center Lake
Co-located with walk-in clinic operations and will have lab and X-ray capabilities on-site
Team will eventually consist of 4 primary care providers
