About the role
The RN Outpatient Case Manager serves as a bridge between the healthcare professional in the hospital setting and the patient and their caregiver. They provide information and guidance for an effective care transition, improved self-management skills, and better patient-physician communication.
Responsibilities
- Provide education to patients and caregivers about care transition, self-management, and referrals to other healthcare resources.
- Identify and address critical issues and self-management tasks for patients and caregivers.
- Work with healthcare professionals to ensure smooth transitions from hospital to home and other post-acute care settings.
- Ensure patients and caregivers have the necessary knowledge and support to manage their health effectively.
Requirements
- Graduation from an accredited school of nursing.
- At least 5 years of experience as an RN.
Qualifications
- Ability to think critically and analytically with little or no supervision.
- Effective communication skills, both verbal and written.
- Strong clinical knowledge and expertise.
- Independent work habits and self-reliance.
- Basic computer literacy.
