Orthopedics Care Coordinator RN

This is a Full Time Day shift position, Monday - Friday.

Responsibilities

• Oversee the overall care of orthopedic/Joint patients following a holistic and interdisciplinary approach in collaboration with medical staff, patients/family members, and other members of the health care team.
• Serve as a leader for the overall program management, process improvement plans, patient satisfaction plans, and communicate goals and status to staff and other team members.
• Coordinate and drive the monthly performance Interdisciplinary meeting. Collect, analyze, and report quality metrics data with team members, plan interventions, and ensure appropriate action/process is in place to achieve outcomes/goals.
• Responsible for coordinating and providing education for patients and families before surgery (Pre-op classes) and before discharge (discharge education). Coordinate teaching materials for patients/families and physician’s office.
• Collaborate and facilitate staff education on various aspects of orthopedic patient care to assist the team following best practices/clinical practice guidelines in alignment with current process/protocol.
• Make daily clinical rounds on patients, assess therapy and other needs of patients from admission through discharge, and coordinate care needs with other team members.
• Perform daily rounding with physicians, provide updates on patient progress, challenges, and need for interventions to improve patient care, treatments, and overall experience.
• Conduct concurrent chart reviews to obtain data and analyze documentation of clinical components that meet joint protocol/standards.
• Collaborate with other disciplines to ensure discharge needs are met, including discharge education, after-discharge therapy, HHC, DMEs, and follow-up appointments.
• Collaborate/create measurement systems for tracking surgical outcomes, patient satisfaction, and identify areas that require improvement.
• Oversee, plan, and develop formal and informal educational materials for patients/families and ensure educational needs are met. Coordinate teaching materials with physician’s office.
• Act as a liaison to patients/families, physicians/office staff, and other team members involved in orthopedic patient care/treatment.
• Coordinate patient post-discharge calls to ensure continuum of care pathways are followed/met.
• Coordinate/collaborate on community outreach programs to involve the community with educational opportunities.
• Coordinate all aspects required for recertification of the joint program with The Joint Commission and any other accreditation bodies.

Qualifications

• Graduate of an accredited school of Professional Nursing.
• BSN is required; Advanced Practice Nursing/Masters prepared preferred.
• Current RN license in the State of TX or compact license.
• Orthopedic Nurse Certified.
• Minimum of 5 years recent clinical nursing experience in orthopedics.
• Recent experience in Orthopedic outcomes research and intervention is desirable.
• Current BLS issued by the American Heart Association or the American Red Cross.
• Current ACLS preferred, issued by the American Red Cross or the American Heart Association.
