About the role
JAG Physical Therapy is seeking an experienced Orthopedic At-Home Physical Therapist to provide care to patients recovering from total hip and knee replacement surgeries and other orthopedic conditions. The ideal candidate will have a strong background in orthopedic rehabilitation and a passion for helping patients regain mobility and independence in the comfort of their own homes.
Responsibilities
- Educate patients on safe bed mobility, transfers, and ambulation considering post-operative precautions.
- Transfer, ambulate, and perform stair negotiation with patients to ensure they are safe for discharge to home.
- Collect and record patient care data following department policies and standards of practice.
- Prepare for evaluations according to department policies and procedures, including obtaining patient history and researching unfamiliar diagnoses.
- Select and administer tests and measures, synthesize evaluation results to formulate diagnosis, goals, prognosis, and plan of care.
- Establish and modify treatment plan based on evaluation results, goals, and best practices.
- Collaborate with interdisciplinary team, patients, and patient's support system.
- Provide education and training to patients and/or patient support system on functional training, ADLs/IADLs, therapeutic activities, rehabilitation equipment prescription, and community reintegration.
- Prescribe and train on rehabilitation equipment.
- Provide interventions that are safe, effective, and compliant with JAG standards.
- Communicate with Manager/Patient Access team for scheduling evaluations once demographics are received.
Requirements
- Bachelor’s or Master’s Degree required, Doctorate degree preferred.
- Licensed Physical Therapist in the state practicing in.
- Completed state regulated CEU requirement in appropriate state (NY, NJ).
- Valid CPR License.
- Experience/knowledge with an EMR software is preferred.
- Excellent oral, written communication skills.
- Strong clinical decision-making skills.
- Excellent work ethic and dedication to patient success.
- Basic computer skills.
- Works as a team member and individually with minimal supervision.
- Maintains a positive, professional demeanor at all times.
Qualifications
