OPWDD Community Marketing Representative

About the role

Castle Rock Home Care is a growing provider of pediatric and developmental disability services. We are seeking an experienced and relationship-driven OPWDD Community Marketing Representative to expand our presence throughout New York City and build meaningful partnerships within the developmental disability community.

Responsibilities

• Develop and maintain relationships with OPWDD Care Managers, CCOs, schools, hospitals, clinics, social workers, and community organizations.
• Educate referral sources on available home care, nursing, therapy, and support services.
• Attend networking events, conferences, resource fairs, and community outreach initiatives.
• Serve as a trusted resource for families navigating OPWDD eligibility and service coordination.
• Collaborate closely with intake, eligibility, and clinical teams to ensure smooth referral conversion.
• Maintain accurate referral tracking and pipeline management within CRM systems.
• Identify new business opportunities and strategic partnerships throughout NYC.

Qualifications

• Must have active OPWDD care management experience.
• Experience in home care, healthcare marketing, care coordination, or business development.
• Strong understanding of the OPWDD system and developmental disability service landscape.
• Existing relationships with Care Managers, CCOs, schools, or community organizations are preferred.
• Excellent communication and relationship-building skills.
• Self-motivated, organized, and comfortable working independently in the field.
• CRM experience preferred.

Performance Expectations

Successful candidates will be expected to build and manage a robust referral network capable of generating consistent monthly growth. The target for this role is 10 qualified Starts of Care (SOCs) per month, achieved through community outreach, referral source development, and strategic relationship management.
