Responsibilities
- Answer and manage live triage and physician telephone lines, ensuring timely and accurate communication with clinical staff.
- Assess incoming calls and appropriately route information from physicians, patients, caregivers, and family members to the appropriate clinical manager or team member.
- Conduct wellness calls to patients for appointment reminders, follow-up care, patient education, and general well-being checks.
- Coordinate, document, and communicate laboratory results to clinicians, physicians, patients, and family members as directed.
- Serve as a communication liaison between patients, referral sources, physicians, and internal clinical teams.
- Maintain accurate and timely documentation within agency systems.
- Support quality assurance initiatives and maintain established service standards.
- Assist in reviewing, analyzing, and tracking clinical referral and patient data as needed.
- Ensure compliance with all agency policies, regulatory requirements, and quality standards.
- Perform additional duties and special projects as assigned.
Requirements
- Current Massachusetts LPN License in good standing
- Graduate of an accredited Licensed Practical Nursing (LPN) program
- Minimum of five (5) years of recent nursing experience
- Minimum of three (3) years of community health or home health experience preferred
Qualifications
- Knowledge of Medicare and Managed Care regulations and processes
- Strong organizational and time-management skills
- Excellent verbal and written communication skills
- Outstanding customer service and interpersonal skills
- Ability to effectively prioritize, problem-solve, and multitask in a fast-paced environment
- Strong analytical and troubleshooting abilities
- Proficiency with computers, electronic medical records, and keyboarding
- Ability to communicate professionally and compassionately with patients, families, physicians, and staff

Pay
Compensation: 35.00 - 42.00 per hour based on experience
Schedule
Full-Time, 40 hours per week, onsite in Norwell
