About the Role

The Nurse Navigator will serve as a single point of contact for referring physicians, patients, and caregivers to provide resources and assistance to patients. They will assist in overcoming treatment barriers while expediting access to clinical and supportive care services offered within the Jupiter Medical Center’s Program, and the community. The Nurse Navigator will prepare and maintain an updated treatment plan for each patient to synchronize care management between the medical team and patient. Assistance with patient appointments, including those made with labs, diagnostic facilities, and specialty physicians will be provided. Community outreach including possible public speaking and promotion of the program and Jupiter Medical Center system is an essential part of this position. Performs other duties as assigned.

Responsibilities

  • Serve as a single point of contact for referring physicians, patients, and caregivers to provide resources and assistance.
  • Assist in overcoming treatment barriers and expedite access to clinical and supportive care services.
  • Prepare and maintain an updated treatment plan for each patient to synchronize care management.
  • Coordinate patient appointments with labs, diagnostic facilities, and specialty physicians.
  • Engage in community outreach, including public speaking and promotion of the program and Jupiter Medical Center.
  • Perform other duties as assigned.

Requirements

  • Graduate of an accredited school of professional nursing; BSN preferred.
  • Current RN licensure in the state of Florida.
  • American Heart Association’s BLS Certification for Healthcare Providers.
  • Five (5) years of nursing experience with three (3) in Oncology related field preferred.
  • Knowledge of physician office processes.
  • Excellent verbal and written communication skills.
  • Proficient in building relationships and coordinating all aspects of care.
  • Knowledgeable of oncology disease process and symptom management.
  • Ability to foster and maintain effective working relationships and communication with patients, employees, physicians, administration, and the general public.
  • Performs independently within scope of practice.
  • Ability to interpret, recognize, and analyze difficulties in patient care situations.
  • Exhibits excellent leadership, organizational, written, and verbal communication and interpersonal skills.

Physical Requirements

  • Stooping, sitting, standing, and walking for long periods of time.
  • Frequently assisting with lifting patients and heavy equipment.
  • Ability to work in a stressful environment, work independently, make sound decisions, and remain detail-oriented, alert, and self-motivated.
  • Ability to push/pull up to 40 lbs, carry and lift up to 50 lbs.

Threshold Requirements

  • Annual Joint Commission mandatory education requirements, in-service, and health requirements including attendance at new employee orientation.
  • TB/PPD Surveillance Program.
  • Maintenance of required professional licensing and/or certification(s).
