Uses oncology-specific clinical knowledge to offer individualized guidance to patients, families, and caregivers, fostering independence during their complex cancer journey. Creates a patient-centered, compassionate environment of care that inspires and motivates.

Responsibilities

  • Participates in day-to-day operations of the program including patient education and call triage.
  • Participates in program evaluation and development of strategic goals consistent with Dana-Farber Cancer Institute’s mission and professional values.
  • Participates in QA/QI projects for the program and supports research activities.
  • Exhibits professional commitment through integrity, honesty, and accountability.
  • Delivers expert clinical care through evidence-based practice partnerships with patients, families, and colleagues.
  • Assumes nursing care responsibilities for a defined group of patients, including assessment, education, coordination, and documentation.
  • Facilitates individualized care considering functional status, cultural context, health literacy, psychosocial, reproductive/fertility, and spiritual needs.
  • Supports smooth transitions from active treatment into survivorship, chronic cancer management, or end-of-life care.
  • Assists patients with treatment goals, advanced directives, palliative care, and end-of-life concerns using an ethical, nonjudgmental framework.
  • Acts as a liaison between patients, families, caregivers, and providers to optimize outcomes.
  • Advocates for patients to promote shared decision-making and patient-centered care aligned with patient goals.
  • Provides psychosocial support and facilitates appropriate referrals for patients, families, and caregivers.
  • Contributes to ONN program and role development, implementation, and evaluation within the healthcare system and community.
  • Participates in tracking and monitoring metrics and outcomes to document and evaluate the navigation program.
  • Functions as the primary nursing contact for care delivered within the physician practice setting.
  • Engages in collaborative decision-making using available data and collective expertise.
  • Manages patient care across the continuum, including new patient calls and making appropriate referrals.
  • Works collaboratively as an effective member of the healthcare team.
  • Monitors the environment of care with attention to patient safety and regulatory compliance.
  • Maintains evidence-based clinical practice consistent with nursing policies, procedures, and standards.
  • Provides age-specific care and adheres to the Code of Ethics for Nurses.
  • Complies with licensure/registration requirements and professional nursing standards.
  • Fosters an environment sensitive to diverse populations, including culture, ethnicity, gender, and age.
  • Identifies areas for professional growth and formulates goals and methods for accomplishment.
  • Serves on Nursing and Institute committees, including the Program Nurse Committee.
  • Supports research activities within Nursing and Patient Care Services and the Institute.
  • Meets annual evaluation requirements and competencies.
  • Collaborates in preparing professional reports, articles, and presentations.
  • Pursues active membership in local and national professional organizations.
  • Demonstrates commitment to continuous learning, improvement, and mentoring.

Requirements

  • Associate degree in Nursing; Baccalaureate degree in Nursing strongly preferred.
  • If Associate degree in Nursing, must complete BSN degree within 4 years.
  • 1 year of recent experience as a Registered Nurse; ambulatory and/or oncology experience preferred.
  • Licensed as a Registered Nurse in the Commonwealth of Massachusetts or State of New Hampshire.
  • AHA BLS required (AHA ACLS accepted as minimum requirement).
  • Professional nursing certification preferred (OCN, BMTCN, CPHON, AOCNP, CRNI, RN-BC).

Pay

$44.14 - $109.51 per hour.
