CareMed is a unique Pharmacy model serving community, oncology, and hematology physicians, patients, payers, and manufacturers. We are seeking an Oncology Nurse Navigator to join our growing team. The position is remote but requires living within commuting distance to our pharmacy in Louisville, KY. Starting salary is $80K and above with a sign-on bonus of $5,000 for those starting before July 31, 2026.

Responsibilities

• Communicate with patients to help them understand the disease, treatment plan, and possible side effects.
• Provides medication therapy management.
• Assists health care providers and patients, greeting them by phone, answering questions, and requests.
• Reviews medical and health history of patients.
• Communicates with adult patients and physician's office staff.
• Complies with professional practice and patient confidentiality laws.
• Provides medication therapy management.
• Dedicated rare disease support and management of rare disease patients.
• Documents any adverse drug reactions according to manufacturer guidelines.
• Informs pharmacy manager of medication errors.
• Documents all communication with physicians, or other healthcare providers.
• Documents adverse events and product complaints per contractual obligations to pharmaceutical manufacturers.
• Follows up with adult patients and providers to monitor progress towards meeting drug treatment goals.

Qualifications

• Education/Learning Experience: Associate of Science in Nursing (Required)
• Work Experience: 1+ years in oncology nursing setting (Required), 1+ years in specialty pharmacy setting (Desired)
• Licenses/Certifications: Current license as registered nurse (Required), Desired: Pediatric nursing certification, Desired: ONCC or ONS certification
• Skills/Knowledge: Compassion, good communication skills, critical thinking, adaptability, attention to detail (Required), Clinical and/or case management experience (Desired)
