As a Per Diem On-Call Weekends Hospice RN (field-based in the San Gabriel Valley areas), you’ll guide patients and families through one of life’s most vulnerable transitions, delivering comfort, dignity, and compassionate care where it matters most—at home.

Pay

$50/hour + mileage reimbursement.

Responsibilities

• Lead and coordinate individualized hospice care plans
• Provide skilled nursing care including pain and symptom management
• Collaborate with physicians and the interdisciplinary team
• Educate and support patients and families during critical moments
• Make a real impact every single day

Requirements

• Licensed Registered Nurse (RN)
• BSN/MSN preferred
• Experienced in hospice, home health, or case management (preferred)
• Compassionate, independent, and confident in the field
• Comfortable with EMR/documentation
