About the role
This role is essential in providing after-hours triage, clinical support, and urgent patient visits across the hospice service area.
Responsibilities
- Answering and triaging all after-hours hospice calls
- Providing clinical guidance to patients, families, and facility staff
- Making urgent or necessary visits after hours when required
- Supporting patients across multiple care settings
Requirements
- Current Registered Nurse license in the state of Texas
- Minimum of one year of experience in hospice experience preferred
- Strong clinical assessment and decision-making skills
- Compassionate, patient-centered approach to care
- Valid State Driver's License and reliable transportation
- Able to work independently and manage time effectively
