Job Summary
On-Call LPN - GREEN BAY, WI
Job Requirements
- Responsible for providing nursing care to clients on assigned area.
- Supervises/directs subordinate nursing personnel - e.g. nursing assistants per RN directive.
- Assists the RN in a variety of client care activities.
- Completes delegated work-related tasks as assigned by the RN.
- Administers and documents medications and treatments on the electronic MAR as prescribed by physicians and according to established nursing policy/procedure.
- Makes rounds with physician and completes order entry in Avatar under supervision of the Registered Nurse.
- Maintains current information on new medications, dosages and side effects.
- Provides medication teaching with client/family as indicated in client care plan and as directed by the Registered Nurse.
- Provides nursing care to meet the physical, emotional and psycho-social needs of the client.
- Participates in the development of client care plans.
- Implements provisions of client care plan.
- Observes and documents change in client behavior/condition and inform RN accordingly.
- Participates in emergency or crisis situations as directed by the Registered Nurse.
- Affords assistance with admissions, discharges and transfer of clients.
- Maintains records, reports and documentation in accordance with departmental standards.
- Maintains a clean, comfortable and safe environment for clients.
- Assists in creating a therapeutic atmosphere on the client unit.
- Maintains effective working relationships with other employees.
- Attends and participates in staff meetings, mandatory in-services and other educational programs.
- Perform related functions as assigned.
Physical Demands
- Lifting 75 pounds maximum with the assistance of another person, with frequent lifting and/or carrying of objects weighing up to 40 pounds.
- Extended periods of standing, walking and sitting;
- Sometimes driving.
- Using hand(s) for repetitive single grasping, fine manipulation, pushing and pulling, and operating controls.
- Frequent bending, twisting, squatting, climbing, reaching and grappling;
- Sometimes swimming.
- Communicating orally in a clear manner.
- Distinguishing sounds at various frequencies and volumes.
- Distinguishing people or objects at varied distances under a variety of light conditions.
- Must be free from communicable disease.
Other Experience and Qualifications
- Knowledge and understanding of the terminology, techniques and practices of Practical Nursing.
- Knowledge of basic needs of the geriatric elderly, developmentally disabled, mentally ill, AODA client and child/adolescent clients.
- Knowledge of basic pharmacology and specifically psychopharmacology (expected actions, side effects, contraindications, and route of administration, specifically types of injectables).
- Knowledge of principles of documentation.
- Knowledge of basic principles of behavior modification and appropriate application of behavior modification techniques.
- Ability to complete basic Medicare evaluations following completion of proper training and return demonstration of competency.
- Ability to recognize and address the physical, psycho-social, mental and emotional needs of the client.
- Ability to establish and maintain therapeutic relationships with clients and their family members.
- Ability to carry out a nursing care plan as directed by the Registered Nurse.
- Ability to maintain accurate records.
- Ability to enter physician orders into the electronic record system under the supervision of the Registered Nurse.
- Ability to use all standard medical equipment.
- Ability to perform in emergency situations as directed by the Registered Nurse.
- Ability to recognize and maintain confidentiality of all matters related to client care.
- Ability to understand and carry out policies and procedures governing client care.
- Ability to participate in unit activities and to serve as a role model for clients.
- Ability to function within the organizational structure utilizing the chain of command.
- Ability to follow oral and written instructions.
- Ability to establish and maintain effective interpersonal working relationships.
- Ability to communicate effectively both orally and in writing.
- Ability to work the required hours of the position.
