Responsibilities
- Respond to on-call messages, making recommendations via phone and making visits as needed
- Complete comprehensive assessments of patients and families to determine hospice eligibility and care needs
- Provide skilled nursing care, including education and support to patients and loved ones
- Complete clinical notes regarding patient condition and care given
- Supervise hospice Certified Nursing Assistants (CNAs)
- Teach patients and loved ones self-care techniques as appropriate
- Provide emotional support and bereavement support to patients and their families
- Initiate and revise plans of care as patient status and needs change
- Assess and evaluate patient status, identifying opportunities for improved care and quality of life
Requirements
- Graduate of an accredited school of nursing
- Current and active RN license in Massachusetts; CPR certification
- 1-2 years of experience in acute care, home care, or hospice preferred
- Strong communication, problem-solving, and basic math skills
