On-Call Hospice Nurse

About the role

Provide compassionate home-based care focused on quality of life for patients with limited prognosis. Perform patient assessments, monitor vital signs, manage pain, administer medications, and ensure access to necessary medical equipment. Collaborate with an interdisciplinary team to educate families about prognosis, care plans, and hospice goals, taking a holistic approach to support the patient’s physical, emotional, and spiritual needs.

Requirements

• Active RN license in Michigan
• At least one year of relevant experience
• Reliable transportation, valid driver’s license, and current insurance

Qualifications (Preferred)

• Community health experience
• Medical-Surgical experience
