Responsibilities

• Provide geriatric house calls to patients in their homes and senior living communities.
• Assess and treat physical and occupational therapy needs.
• Collaborate with interdisciplinary teams to develop and implement care plans.
• Document progress and communicate with patients, families, and other healthcare providers.

Requirements

• Current licensure to practice physical therapy in at least one state.
• Experience in geriatric care preferred.
• Strong interpersonal skills and ability to work effectively with diverse populations.
• Passion for making a difference in the lives of older adults.

Qualifications

• Master's degree in Physical Therapy or Occupational Therapy.
• ACOTE accredited program.
• Current CPR certification.
• Excellent communication and problem-solving skills.

Skills

• Strong clinical skills in physical and/or occupational therapy.
• Ability to work independently and as part of a team.
• Effective time management and organization skills.
• Proficiency in Microsoft Office Suite.
