About the role

Sobe Innovative Rehab is seeking a licensed Occupational Therapist to provide outpatient therapy services in a home setting in Polk City, FL. Join our team and help older adults maintain their independence and quality of life through compassionate, evidence/comfort-based care.

Responsibilities

• Evaluate and treat residents in a home setting
• Develop and implement individualized treatment plans
• Collaborate with physicians, families, and the interdisciplinary care team
• Maintain accurate, timely documentation in the EMR

Qualifications

• Bachelor's, Master's, or Doctorate degree in Occupational Therapy from an accredited program
• Active Florida Occupational Therapist (OT) license
• New graduates and experienced therapists are encouraged to apply

Schedule

Flexible PRN schedule – choose the assignments that fit your availability. PRN therapists work on an as-needed basis, selecting the assignments they wish to accept, offering flexibility and control over your schedule.
