Occupational Therapist OT PRN

As an Occupational Therapist in Home Health, you’ll play a key role in delivering high-quality care to patients in their preferred home setting. You’ll contribute to our mission by evaluating patients, developing personalized care plans, and ensuring continuity of care while working collaboratively with a team of healthcare professionals.

Responsibilities

• Evaluates occupational therapy patients to determine rehabilitation needs, potential for achievement, and develops a plan of care.
• Participates in the implementation and development of the Plan of Care to ensure quality, continuity of care, and proper discharge planning.
• Makes the initial therapy evaluation visit and re-evaluates patients’ therapy needs during each visit, following OASIS and PPS regulatory guidelines.
• Communicates significant findings, problems, or changes in condition to supervisors, physicians, and other team members.
• Initiates plans for patient safety using patient, family, and community resources.
• Verifies the Plan of Care prior to each visit and provides care according to physician’s orders, assessment data, and established standards.
• Writes physician orders for additional visits or changes to the Plan of Care, per agency policy.
• Incorporates patient care goals from the Plan of Care into therapy, as documented in therapy notes.
• Performs occupational therapy services in accordance with accepted standards of practice and physician certification.
• Instructs patients and families in “activities of daily living” using adaptive equipment, sensory integration, and fine motor coordination skills.
• Assists patients in securing necessary equipment, including fabrication of custom-made splints.
• Instructs patients and caregivers in therapy home programs, providing modifications and clarifications as needed.
• Counsels and instructs patients and families in meeting therapy and related needs.
• Supervises and provides clinical direction to Certified Occupational Therapy Assistants (COTA), Home Health Aides (HHA), and clinical staff to ensure the therapy care plan is followed.

This role operates in a physically demanding, high-stress environment with a full range of body motion, including handling, lifting, and transferring patients. Potentially irregular hours, including on-call, may be required.

Requirements

• Graduate of an accredited Occupational Therapy program.
• Current, unrestricted OT license valid for the state of work.
• Minimum one (1) year of experience in a clinical care setting.
• Able and willing to travel within the branch/office coverage area.
• Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
• Must be able and willing to travel up to 50% of the time.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.

You will report to the Branch Director, Clinical Manager, Clinical Supervisor, or designee.
