Occupational Therapist (OT) In House SNF (FT PT or PRN)

About the role

Evolve Therapy Services is hiring compassionate occupational therapists to provide in-house services at a skilled nursing facility in Erie, PA. The role involves planning, organizing, and directing Occupational Therapy Services in accordance with federal, state, and local standards to ensure the highest degree of quality resident care.

Responsibilities

• Conduct regular screenings of residents to determine the best intervention or treatment for each individual.
• Evaluate residents to develop and inform treatment plans, including short- and long-term goals and methods to achieve them.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules in cooperation with other team members.
• Conduct therapy according to established treatment plans and ensure proper documentation and billing of occupational therapy services.
• Oversee Certified Occupational Therapy Assistants (COTA) as needed, ensuring clinical excellence and proper reimbursement practices.
• Perform all other duties as assigned to effectively treat patients.

Requirements

• Degree in Occupational Therapy.
• Active Occupational Therapy license in the practicing state(s).
• Ongoing continuing education and professional development to maintain licensure and certification.
