Occupational Therapist (OT) In House SNF (FT or PRN)

About the Role

Evolve Therapy Services is hiring compassionate therapists for our partner located in State College, PA. We are seeking an Occupational Therapist for an In-house Skilled Nursing Facility. Full-time or PRN positions are available.

Job Purpose

To plan, organize, develop, and direct Occupational Therapy Services in the facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations. This role ensures the highest degree of quality resident care is maintained at all times, as directed by the Administrator.

Responsibilities

• Conduct regular screening of residents to determine the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plans and ensure proper documentation and billing of occupational therapy services
• Oversee COTA as needed, ensuring all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are followed
• Perform all other duties as assigned and required to effectively treat patients

Qualifications

• Degree in Occupational Therapy
• Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
