Occupational Therapist (OT) In House PRN

About the role

Evolve Therapy Services is seeking a compassionate Occupational Therapist to provide in‑house skilled nursing facility services in Rice Lake, WI. The therapist will plan, organize, develop, and direct occupational therapy services in accordance with federal, state, and local standards, ensuring the highest quality resident care.

Responsibilities

• Conduct regular screening of residents to determine the best intervention or treatment for each individual patient.
• Evaluate residents to develop treatment plans; establish short‑ and long‑term goals and methods to achieve those goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules in cooperation with other team members.
• Provide therapy according to the established treatment plan and ensure proper documentation and billing of occupational therapy services.
• Perform all other duties as assigned and as required to effectively treat patients.
• Oversee Certified Occupational Therapy Assistants (COTA) as needed, ensuring documentation and treatments meet clinical excellence and reimbursement standards.

Qualifications

• Degree in Occupational Therapy.
• Active Occupational Therapy license in the practicing state(s).
• Ongoing continuing education and professional development to maintain current licensure and certification.
