Job Summary

About the Role

As a Home Health OT, you will provide occupational therapy to patients unable to perform daily tasks due to illness or injury. You will work as part of a home health team, including an RN, LPN, CNA, HHA, PT, and SLP, to ensure the patient's plan of care and personal goals are met. Your responsibilities include assessing patients, establishing therapy goals, documenting progress, assisting with exercises, suggesting adaptive equipment, assessing fall risks, and educating patients and families.

Responsibilities

• Provide occupational therapy to patients unable to perform daily tasks due to illness or injury
• Work as part of a home health team including an RN, LPN, CNA, HHA, PT, and SLP
• Achieve and document therapy goals
• Assist patients with exercises to improve fine motor skills and coordination
• Suggest adaptive equipment such as grab bars and shower chairs to provide added support
• Assess fall risks and introduce strategies to improve home safety
• Educate patients and families on plan of care, exercises, goals, and self-care

Requirements

• Graduate of an accredited Occupational Therapy Program and active OT license in CO
• Minimum of 3 years of occupational therapy experience, ideally in home healthcare
• CPR certification
• Knowledge of state and federal home health regulations
• Good clinical judgment, strong interpersonal skills, resourcefulness, and compassion
