Key Responsibilities
- Conduct comprehensive patient evaluations and develop individualized treatment plans.
- Provide skilled occupational therapy interventions in the home setting.
- Educate patients and families on therapeutic techniques and safety.
- Monitor patient progress and document outcomes in the EMR.
- Communicate effectively with patients, caregivers, and the interdisciplinary care team.
- Ensure compliance with home health regulations, physician orders, and documentation standards.
- Participate in care coordination and quality improvement initiatives.
- Collaborate with clinical and administrative teams to ensure excellent patient care.
- Perform additional duties as assigned to support patient outcomes.
Qualifications
- Master’s or Doctorate Degree in Occupational Therapy
- Active state licensure as an Occupational Therapist
- 1–3 years of clinical experience in home health, outpatient, or rehabilitation preferred
- Strong patient assessment and treatment planning skills
- Knowledge of home health regulations and documentation requirements
- Experience with EMR systems and Microsoft Office
- Excellent communication and interpersonal skills
- Ability to work independently and manage a patient caseload

- Flexible scheduling with availability during business hours and occasional after-hours/weekend coverage.
