Occupational Therapist (OT) - Home Health (Per Diem)

Key Responsibilities

• Home Safety & Risk Assessment: Evaluate living environments for fall risks and accessibility challenges. Recommend modifications and adaptive equipment to improve safety.
• Functional Independence Training: Assist patients with essential daily activities such as bathing, dressing, and household tasks. Introduce techniques to conserve energy and reduce strain.
• Personalized Care Planning: Develop individualized therapy programs based on each patient’s condition and home setup. Adjust interventions to reflect progress and changing needs.
• Caregiver Support & Education: Instruct family members and aides on safe assistance techniques. Promote consistency between therapy visits.
• Clinical Documentation: Maintain accurate and timely records using an electronic documentation system. Ensure compliance with all regulatory standards.

Qualifications

• Active New York State Occupational Therapist license.
• Experience with adult or geriatric populations preferred.
• Ability to assess and adapt to varied home environments.
• Strong communication and problem-solving skills.
• Comfortable using EMR systems.
