ABL Health Care and ABL Hospice are dedicated to delivering compassionate, personalized care in home healthcare and hospice services. We are currently seeking a Full Time or Part Time (Per Diem) Occupational Therapist (OT) in Alameda, CA.
Position Overview
We are seeking an experienced, self-driven Full-Time / Part Time (Per Diem) Occupational Therapist (OT) to join our dedicated Home Health team and provide exceptional care throughout the region. As an Occupational Therapist (OT), you will evaluate patients' health, create and implement care plans, and track their progress to maintain the highest standards of care.

Responsibilities
- Provide, coordinate and oversee skilled intermittent care to patients and include instruction and education of patients and/or caregivers to promote maximal functional status, in the patient's home
- Regularly evaluates and re-evaluates patient status
- Administer treatments, monitor patient conditions, and report signs/symptoms signaling a change in patient condition to Physician, Case Manager, and/or as needed Director of Nursing
- Initiates, develops, implements and revises the patient's plan of care (POC)
- Adds addresses all problems identified in the POC
- Provides educational information to the patient/family/caregiver concerning knowledge deficits or needs regarding the patient's condition
- Communicates with other disciplines involved in the case as warranted
- Maintain accurate clinical records and submit documentation according to company policy using a company-provided device
- Comply with all company policies and procedures to ensure high standards of care
Requirements
- Current OT License to practice in California - Required
- Experience in Home Health or Hospice care - 1 Year Required
- Current CA Driver's License
- Able to travel within a 15 - 25 mile radius on a regular basis
