Occupational Therapist (OT) - Home Health

Responsibilities

• Reviewing and communicating care plans with physicians.
• Educating patients and families to overcome barriers.
• Documenting visits, assessments, and care plans accurately and timely.
• Monitoring patient progress, updating goals, and coordinating with other disciplines.
• Ordering supplies, planning discharges, and evaluating home safety.
• Supervising Occupational Therapy Assistants and home health aides.

Qualifications

• Bachelor's degree in Occupational Therapy.
• Licensed Occupational Therapist (IDFPR) in Illinois.
• Basic Life Support (BLS) certification.
• Valid driver’s license and auto insurance.
• Strong teamwork, communication, assessment, and documentation skills.
