As an Occupational Therapist, you’ll play a key role in delivering high-quality home health care to over 60,000 patients daily in their preferred home setting. You’ll contribute to our mission by evaluating patients, developing personalized care plans, and ensuring continuity of care in a collaborative environment.

Responsibilities

• Evaluate occupational therapy patients to determine rehabilitation needs, potential for achievement, and develop a plan of care.
• Participate in the implementation and development of the Plan of Care to ensure quality, continuity of care, and proper discharge planning.
• Conduct initial therapy evaluation visits and re-evaluate patients’ therapy needs during each visit.
• Make follow-up assessments according to OASIS and PPS regulatory guidelines.
• Communicate significant findings, problems, or changes in condition to supervisors, physicians, and other team members.
• Initiate plans for patient safety using patient, family, and community resources.
• Verify the Plan of Care prior to each visit and provide care according to physician’s orders, assessment data, and established standards.
• Write physician orders for additional visits and changes to the Plan of Care, per agency policy.
• Incorporate patient care goals from the Plan of Care into therapy, as documented in therapy notes.
• Perform occupational therapy services in accordance with accepted standards of practice and physician certification.
• Instruct patients and families in “activities of daily living” using adaptive equipment, sensory integration, and fine motor coordination skills.
• Assist patients in securing necessary equipment, including fabrication of custom-made splints.
• Instruct patients and caregivers in the therapy home program, providing modifications and clarifications as needed.
• Counsel and instruct patients and families in meeting therapy and related needs.
• Supervise and provide clinical direction to Certified Occupational Therapy Assistants (COTA), Home Health Aides (HHA), and clinical staff to ensure the therapy care plan is followed.
• Operate in a physically demanding, high-stress environment with full range of body motion, including handling, lifting, and transferring patients.

Requirements

• Graduate of an accredited Occupational Therapy program.
• Current, unrestricted OT license valid for the state of work.
• Minimum one (1) year of experience in a clinical care setting.
• Able and willing to travel within branch/office coverage area.
• Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
• Must be able and willing to travel 50% of the time.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
