About the Role
As a full-time Occupational Therapist, you’ll enjoy a consistent weekday schedule while helping patients regain independence and improve their quality of life in the comfort of their homes. Responsibilities
- Evaluate patients to identify occupational therapy needs and develop individualized Plans of Care
- Complete initial evaluations, reassessments, and follow-up visits in compliance with OASIS and PPS guidelines
- Implement and revise Plans of Care to ensure quality, continuity, and effective discharge planning
- Communicate significant findings and changes in patient condition to physicians and interdisciplinary team members
- Provide occupational therapy services in accordance with physician orders and accepted standards of practice
- Instruct patients and caregivers in activities of daily living, adaptive equipment use, sensory integration, and fine motor coordination
- Assist patients and caregivers in obtaining necessary equipment, including fabrication of custom splints when appropriate
- Educate patients and families on home therapy programs and modify plans as needed
- Supervise and provide clinical direction to COTAs, HHAs, and clinical staff to ensure high-quality care delivery
- Promote patient safety using family and community resources
Requirements
- Graduate of an accredited Occupational Therapy program
- Current, unrestricted OT license in the state of practice
- Minimum one (1) year of experience in a clinical care setting
- Reliable transportation, valid driver’s license, and current auto insurance
- Ability and willingness to travel within the branch coverage area (up to 50%)
Physical & Work Requirements
- Ability to sit, stand, bend, lift, and move intermittently
- Ability to lift and transfer 50–100 pounds
- Home-based, physically active work environment
- May include occasional irregular hours or call coverage, as applicable
