Occupational Therapist-OT Full-Time
About the role
Garden City Health Care Center is seeking an Occupational Therapist (OT) to join our dedicated rehabilitation team. We are committed to helping residents achieve their highest level of independence and quality of life through patient-centered, evidence-based care.
Responsibilities
- Evaluate residents within 24 hours (or one business day) of physician referral; document any delays appropriately
- Develop individualized treatment plans and obtain physician approval
- Treat residents according to physician-approved plans of care
- Supervise Certified Occupational Therapy Assistants (COTAs), aides, and students in compliance with state practice act
- Assist nursing staff with training of Restorative Aides as appropriate
- Communicate resident progress, challenges, and plans with the rehab and interdisciplinary teams
- Participate in patient care conferences, utilization review meetings, and rehab conferences as needed
- Complete evaluations, daily treatment notes, weekly progress notes, recertifications, and discharge documentation per regulatory and payer requirements
- Education and in-services to facility staff to support optimal resident outcomes
- Educate residents, families, and caregivers on maintenance programs and discharge preparation
- Participate in discharge planning and care coordination
- Recommend and facilitate ordering of durable medical equipment (DME) to promote independence in activities of daily living
- Report equipment issues to ensure safe and effective operation
- Maintain positive, professional relationships with residents, families, staff, and colleagues
Qualifications
- Bachelor’s degree in Occupational Therapy required
- Master’s or Doctorate degree in Occupational Therapy preferred
- Skilled nursing or geriatric experience preferred
Skills
- Proficient in electronic documentation systems (Casamba and PointClickCare preferred)
- Strong clinical judgment, communication, and organizational skills
- Ability to work independently and collaboratively
Physical Demands
- Frequent standing, walking, reaching, pushing, and pulling
- Ability to exert up to 50 pounds frequently and 100 pounds occasionally
- Occasional climbing, balancing, stooping, kneeling, crouching, or crawling
- Visual abilities include close, distance, color, peripheral, and depth perception
Work Environment
- Skilled nursing and rehabilitation setting
- Noise level typically low to moderate
