Occupational Therapist (OT) for Home Health

About the role

FeldCare Connects is seeking a self-motivated Independent Contractor Occupational Therapist to deliver premier care in the Chino Hills area. This role focuses on working with adults of all ages, assessing and evaluating patient needs related to functionality, activities of daily living, fine motor coordination, home assessments, adaptive equipment, and other occupational therapy needs as defined by medical conditions.

The Occupational Therapist will develop and implement treatment plans, perform interventions to enhance patient well-being, and evaluate progress.

Responsibilities

• Provide services in accordance with state laws governing Occupational Therapy, the plan of care, and in coordination with the healthcare team.
• Lead patients in therapeutic, creative, and self-care activities to improve functioning and safety.
• Observe, record, and report patient responses to treatment and changes in condition.
• Instruct patients, families, caregivers, and the healthcare team on therapy techniques to assist the patient.

Qualifications

• Occupational Therapist license and registration by the state.
• Completion of an accredited Occupational Therapist program.
• Ability and enthusiasm for working with adults of all ages.
• Bilingual skills are a plus.
