Occupational Therapist (OT)
About the role
Lone Tree Post Acute is seeking a compassionate and skilled Occupational Therapist (OT) to join our in-house rehabilitation team. The role involves evaluating residents and developing individualized, evidence-based treatment plans, providing skilled occupational therapy services, supervising and mentoring COTAs and therapy students, participating in care planning meetings and interdisciplinary collaboration, maintaining accurate clinical documentation, and supporting safe and successful discharges through caregiver education and discharge planning.
Responsibilities
- Evaluate residents and develop individualized, evidence-based treatment plans
- Provide skilled occupational therapy services focused on functional outcomes and independence
- Supervise and mentor Certified Occupational Therapy Assistants (COTAs) and therapy students
- Participate in care planning meetings and interdisciplinary collaboration
- Maintain accurate clinical documentation and ensure regulatory compliance
- Support safe and successful discharges through caregiver education and discharge planning
Qualifications
- Current California Occupational Therapist (OT) license
- Strong clinical, communication, and leadership skills
- Commitment to resident-centered care and positive outcomes
