About the role
In this role, you’ll deliver personalized therapy that supports patients in performing daily activities safely and confidently.
Responsibilities
- Provide in-home occupational therapy services to patients and, when applicable, in assisted living settings
- Evaluate patients’ physical, cognitive, and environmental needs
- Develop and implement individualized care plans in collaboration with the healthcare team
- Help patients improve daily living skills, mobility, and overall independence
- Recommend adaptive equipment and home modifications to enhance safety
- Educate patients and caregivers on treatment plans, exercises, and long-term wellness strategies
- Document patient progress and adjust care plans as needed
Requirements
- Active Occupational Therapist license in the State of New Hampshire
- Graduate of an accredited Occupational Therapy program
- Minimum of 2 years of clinical experience (home health preferred)
- Current CPR certification
Qualifications
- Strong clinical judgment, communication, and interpersonal skills
- Ability to work independently and manage a flexible schedule
