Occupational Therapist / OT

About the role

Flagstar Rehab is staffing Occupational Therapists for the Rehab Department in state-of-the-art Skilled Nursing Facilities (SNFs) throughout NYC and surrounding areas. OTs work with a diverse, multidisciplinary team to perform evaluations, treatments, and patient/family education to improve patients' physicality and cognition, positively impacting their ability to function in daily life. Opportunities are available for direct employment with the agency or as an independent contractor, with assistance provided for setting up a corporation free of charge.

Responsibilities

• Provide efficient evaluations and create individualized treatment plans for COTAs to follow.
• Supervise COTAs daily.
• Deliver quality therapy services to individuals in an SNF setting for conditions such as TBI, CVA, stroke, Parkinson’s, multiple sclerosis, cancer, orthopedic surgery, and other debilitating diagnoses.
• Monitor and document patient progress using innovative EMR software.

Requirements

• Current NYS OT License and Registration.
• Competence with EMR systems.
• State-required medical documentation for work in a Skilled Nursing Facility (physical, TB test, MMRV vaccine record or titers, COVID vaccine documentation).
• Valid proof of identity and work authorization.
• Malpractice/liability insurance (if applicable).

Pay

$46–$48 hourly (W-2) or $57–$60 hourly (1099).
