About the Role

As an Occupational Therapist (OT), you will join an interdisciplinary team providing compassionate, home-based therapy care. You’ll work closely with patients and their families, guiding them through every step of the recovery journey while gaining real-world insights into effective patient care.

Responsibilities

  • Provide skilled occupational therapy as directed by the attending physician, including developing and adjusting individualized care plans.
  • Observe, record, and report patient reactions, changes, and progress to the physician and agency staff.
  • Evaluate patient functioning using diagnostic and prognostic procedures.
  • Improve patient outcomes through activities and home exercise programs targeting range of motion, muscle strength, and coordination.
  • Minimize patient risk by educating on preventive measures to reduce re-hospitalizations.
  • Counsel patients and families on managing disease/injury and instruct on proper use of equipment (e.g., wheelchairs, braces, walkers).
  • Supervise and oversee care provided by Certified Occupational Therapy Assistants (COTA).
  • Communicate supervisory visit results to COTAs and collaborate with the care team.
  • Complete accurate clinical documentation in compliance with agency protocols and Medicare/Federal guidelines.
  • Adhere to agency policies, procedures, and regulations, including communicating schedule changes to supervisors.
  • Attend in-service training and mandatory agency meetings.

Requirements

  • Current Occupational Therapist license in the state of employment.
  • Minimum one year of experience as an OT in an acute care setting; home care experience preferred.
  • Valid driver’s license and auto insurance in your name.
  • Capable of meeting the physical demands of the role.
