What You Will Do
- Complete in-home OT evaluations and deliver skilled OT visits.
- Build and execute a clear plan of care focused on ADLs/IADLs, safety/fall prevention, and functional independence.
- Recommend and train on adaptive equipment and home modifications as needed.
- Cookordinate with nursing and the broader care team when condition or needs change.

Requirements
- Active Texas OT license (in good standing).
- Strong clinical judgment and comfort working independently in the field.
- Comfort escalating changes in condition to RN/MD and documenting rationale clearly.
- Strong communication and documentation skills.
- Reliable follow-through on orders, frequencies, and patient goals.
- Current driver’s license and comfort with daily travel across assigned territory.
- Able to carry/transport basic therapy equipment as needed.
- Competency in core home health OT scope (ADL/IADL training, home safety, adaptive equipment, caregiver education).
