Occupational Therapist (OT) - 13 Week Contract

About the role

We are seeking an experienced Home Health Occupational Therapist to provide personalized therapy in patients' homes, helping them regain independence and perform daily activities safely.

Responsibilities

• Assess patients, identify functional deficits, and establish therapy goals.
• Develop and implement individualized treatment plans.
• Work independently while collaborating with a healthcare team.
• Improve patients' fine motor skills, strength, mobility, and coordination.
• Recommend adaptive equipment and assess fall risks to enhance safety.
• Educate patients and families on care plans, exercises, safety, and self-care.
• Complete accurate clinical documentation in compliance with regulations.

Qualifications

• Graduation from an accredited Occupational Therapy program.
• Active OT license in good standing.
• Minimum of one year of home health experience.
• Ability to work independently with limited orientation.
• Strong documentation, time management, and problem-solving skills.
• Proficiency in electronic medical records and knowledge of home health regulations.
• Current CPR certification, valid driver’s license, and reliable transportation.
