About the Role
AYADA Home Health Care is seeking an experienced Occupational Therapist (OT) for a full-time opportunity performing home health visits in the Baltimore area. This position serves adult and geriatric patients on a per-visit basis in their homes throughout Baltimore City. As a home care Occupational Therapist, you will be an integral member of a multi-disciplinary healthcare team providing rehabilitative care and skilled nursing to clients, enabling them to receive necessary medical care while remaining at home.
Responsibilities
- Make home visits to clients in designated geographic territories.
- Perform thorough evaluations of patients’ functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, and ADL/IADL performance.
- Assess cognitive, psychological, and motor aspects of performance and develop individualized intervention plans to achieve favorable outcomes.
- Evaluate home environments for hazards or barriers to independent living.
- Identify equipment needs and environmental adaptations to enhance client outcomes.
- Continually assess and revise the occupational therapy care plan, collaborating with nursing on the multidisciplinary care plan as appropriate.
- Educate and instruct patients, family members, or other representatives in rehabilitative care and activities to promote health, safety, and independent living.
- Accurately document observations, interventions, and evaluations using a state-of-the-art touchpad tablet.
Requirements
- Current Maryland Occupational Therapist license.
- Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA, as indicated by school transcript or diploma.
- Minimum of one year of recent work experience as an Occupational Therapist.
- Prior home care experience preferred but not required.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.

Pay
Base pay ranges from $53.00 to $68.00 per point, depending on qualifications.
