Occupational Therapist Home Health PRN

Job Overview

Talin is seeking a dedicated and compassionate Occupational Therapist to join our team. The ideal candidate will provide high-quality patient care in patients' homes, focusing on improving the functional abilities of individuals with disabilities, injuries, recovery, and aging.

Duties

• Conduct comprehensive evaluations to assess patients' physical, emotional, and social needs.
• Develop and implement individualized care plans tailored to each patient's goals and capabilities.
• Provide direct patient care through therapeutic activities designed to improve daily living skills and overall functionality.
• Collaborate with other healthcare professionals, including speech therapists and case managers, to ensure a holistic approach to patient care.
• Educate patients and their families about treatment options, progress, and strategies for ongoing support.
• Maintain accurate documentation of patient assessments, treatment plans, and progress notes in compliance with policies.
• Monitor patient progress and adjust treatment plans as necessary based on observed outcomes.

Requirements

• Degree in Occupational Therapy from an accredited program.
• Valid state licensure as an Occupational Therapist.
• Strong knowledge of physiology and occupational health principles.
• Experience in case management and working with individuals with disabilities is preferred.
• Familiarity with early intervention strategies is a plus.
• Excellent communication skills to effectively interact with patients, families, and interdisciplinary teams.
• Ability to develop effective care plans that address individual patient needs.
• Career-oriented attitude with a strong commitment to patient care and rehabilitation outcomes.
