Responsibilities

  • Assess and screen patients' daily living and work-related skills, then develop therapeutic retraining programs with measurable objectives.
  • Administer and interpret diagnostic and prognostic tests of function to patients in their homes or other settings to assist physicians in evaluating the patient's level of function.
  • Confer with the patient's physician and other healthcare team members to develop or revise the Plan of Care Treatment.
  • Provide therapeutic treatment and instruction to patients in accordance with physician orders to improve or restore strength, coordination, range-of-motion, and function, or teach compensation measures.
  • Review and expand retraining programs consistent with the behavioral goals of each patient and within the guidelines of the rehabilitation program.
  • Consult with vocational team members to develop and implement vocationally oriented plans consistent with patients' needs and capabilities.
  • Report patient status and progress to physicians, rehabilitation staff, Clinical Managers, patients, and family members.
  • Participate in care coordination and discharge planning activities, acting as a resource for other healthcare personnel.
  • Design community reintegration activities to assist clients in physical reconditioning and psychological adjustment, coordinating with the interdisciplinary team.
  • Recommend or design special adaptive equipment to improve clients' residence or working environments or enhance their participation in the rehabilitation program.
  • Train staff and family members to carry out retraining programs, including dressing, feeding, grooming, and hygiene skills, and participate in team and family educational meetings.
  • Supervise, instruct, and evaluate Occupational Therapy Assistants and Home Health Aides in implementing occupational therapy services.
  • Accurately document patients' care observations, interventions, and evaluations on the day services are rendered, ensuring interim (verbal) orders from physicians are documented and implemented.
  • Submit evaluation, treatment plans, and discharge summaries to supervisors and care management staff.

Requirements

  • Current and unrestricted Occupational Therapy (OT) licensure.
  • Current CPR certification.
  • Valid driver's license, auto insurance, and reliable transportation.

Qualifications

  • Preferred: Minimum six months of occupational therapy experience.
  • Preferred: Home Health experience.

Pay

  • Per Visit/Unit Rate: $54.00 - $76.00.
  • Full-time (40 hours per week) starting base pay range: $85,400 - $117,500 per year. The pay range may vary based on geographic location and individual pay will depend on demonstrated job-related skills, knowledge, experience, education, and certifications.

Additional Information

  • This role is patient-facing and requires Tuberculosis (TB) screening.
  • This role requires a valid state driver's license and maintenance of personal vehicle liability insurance. Minimum required limits: $25,000 bodily injury per person, $25,000 bodily injury per event, and $10,000 for property damage, or higher if required by your state.
  • Scheduled Weekly Hours: 1 (full-time equivalent pay range provided).
