About the role
Provide comprehensive care plans for patients in home health settings, focusing on physical, cognitive, and emotional rehabilitation.
Responsibilities
- Develop and implement individualized treatment plans based on patient needs.
- Conduct assessments to evaluate patient progress and adjust treatment plans accordingly.
- Collaborate with interdisciplinary teams including physicians, nurses, and social workers.
- Document all patient interactions and treatments accurately.
- Ensure compliance with healthcare regulations and standards.
Requirements
- Bachelor’s degree in Occupational Therapy from an accredited program.
- Current license to practice as an Occupational Therapist in Florida.
- At least 1 year of relevant work experience in home health care.
- Excellent interpersonal and communication skills.
- Ability to work independently and as part of a team.
