About the role

Provide comprehensive care plans for patients in home health settings, focusing on physical, cognitive, and emotional rehabilitation.

Responsibilities

• Develop and implement individualized treatment plans based on patient needs.
• Conduct assessments to evaluate patient progress and adjust treatment plans accordingly.
• Collaborate with interdisciplinary teams including physicians, nurses, and social workers.

Requirements

• Bachelor’s degree in Occupational Therapy from an accredited program.
• Current license to practice as an Occupational Therapist in Florida.
• At least 1 year of relevant work experience in home health care.
• Excellent interpersonal and communication skills.
• Ability to work independently and as part of a team.
