Pay
Range: $36.55/hr. to $50.70/hr. (hourly, non-exempt) based on qualifications, experience, and location.

Responsibilities
- Evaluate clients by obtaining and interpreting subjective and objective data
- Develop personalized treatment plans with short-term and long-term goals, ensuring they align with physician orders
- Maintain high standards of care while adapting to different environments and client needs
- Document all services accurately and timely using Vivie’s EMR system
- Demonstrate a thorough understanding of all payer sources and authorization requirements
- Document collaboration with COTAs and perform supervisory visits for HHAs as per regulatory guidelines
- Recommend and oversee continuation programs to support long-term client success and optimize discharge outcomes
- Make referrals to continuation programs as necessary, ensuring clients receive the appropriate follow-up
- Maintain accurate client records and billing
- Communicate effectively and collaboratively with clients, families, referral sources, physicians, and the interdisciplinary team
- Utilize exceptional verbal and written communication skills to ensure cohesive and effective care delivery
- Ensure regulatory compliance by completing and documenting supervisory visits with OTAs and RA/CNAs in accordance with state and regulatory guidelines
Qualifications
- Current Occupational Therapist (OT) license in the state of Minnesota
- CPR certification within 6 months of hire
- Valid driver’s license and access to an insured personal vehicle
- Willingness to float across regions as needed
- Ability to pass state mandated background check
- Strong clinical skills in occupational rehabilitation, skilled therapy services, knowledge of evidence-based occupational therapy practices
- Proficiency in reading, writing, and speaking English to communicate effectively with clients, families, and team members
- Physical capability to perform all job duties
- Experience in home health, long-term care, or rehabilitation settings preferred
