Monday to Friday - 8 a.m. to 4:30 p.m.

Pay

Range: $36.55/hr. to $50.70/hr. (hourly, non-exempt) based on qualifications, experience, and location.

Responsibilities

  • Evaluate clients by obtaining and interpreting subjective and objective data
  • Develop personalized treatment plans with short-term and long-term goals, ensuring they align with physician orders
  • Maintain high standards of care while adapting to different environments and client needs
  • Document all services accurately and timely using Vivie’s EMR system
  • Demonstrate a thorough understanding of all payer sources and authorization requirements
  • Document collaboration with COTAs and perform supervisory visits for HHAs as per regulatory guidelines
  • Recommend and oversee continuation programs to support long-term client success and optimize discharge outcomes
  • Make referrals to continuation programs as necessary, ensuring clients receive the appropriate follow-up
  • Maintain accurate client records and billing
  • Communicate effectively and collaboratively with clients, families, referral sources, physicians, and the interdisciplinary team
  • Utilize exceptional verbal and written communication skills to ensure cohesive and effective care delivery
  • Ensure regulatory compliance by completing and documenting supervisory visits with OTAs and RA/CNAs in accordance with state and regulatory guidelines

Qualifications

  • Current Occupational Therapist (OT) license in the state of Minnesota
  • CPR certification within 6 months of hire
  • Valid driver’s license and access to an insured personal vehicle
  • Willingness to float across regions as needed
  • Ability to pass state mandated background check
  • Strong clinical skills in occupational rehabilitation, skilled therapy services, knowledge of evidence-based occupational therapy practices
  • Proficiency in reading, writing, and speaking English to communicate effectively with clients, families, and team members
  • Physical capability to perform all job duties
  • Experience in home health, long-term care, or rehabilitation settings preferred
