About the role

Spend more time helping patients and less time charting with our AI-assisted documentation tools and the industry-leading WellSky EMR. We offer full-time, part-time, and PRN opportunities in Jacksonville, FL and surrounding areas.

Responsibilities

• Perform occupational therapy evaluations and develop individualized plans of care
• Help patients improve their ability to safely perform activities of daily living and regain functional independence
• Assess home safety, recommend adaptive equipment, and provide caregiver education
• Collaborate with physicians, nurses, therapists, and other members of the interdisciplinary care team
• Complete accurate and timely documentation while maintaining Medicare and agency standards

Requirements

• Current Florida Occupational Therapist (OT) license
• Valid driver’s license and reliable transportation
• Excellent communication and organizational skills
• Home health experience preferred, but not required
• Experience with WellSky or another home health EMR is a plus
