About the role

As a Home Health Occupational Therapist at CenterWell Home Health, you’ll deliver personalized rehabilitation care in patients’ homes, helping them regain strength, mobility, and independence. Reporting to the Home Health Branch Administration, you’ll assess functional abilities, design therapeutic retraining programs, and collaborate with physicians and interdisciplinary teams to improve patients’ quality of life.

Responsibilities

  • Assess and screen patients’ daily living and work-related skills; develop measurable therapeutic retraining programs.
  • Administer and interpret diagnostic and prognostic functional tests to support physician evaluations.
  • Confer with physicians and healthcare team members to develop and revise individualized Plan of Care Treatment.
  • Provide therapeutic treatment and instruction per physician orders to restore or compensate for lost function.
  • Review and expand retraining programs aligned with each patient’s behavioral goals and program guidelines.
  • Consult with vocational team members to implement vocationally oriented plans based on patient needs and capabilities.
  • Report patient status and progress to physicians, rehabilitation staff, Clinical Manager, patients, and family members.
  • Participate in care coordination, discharge planning, and interdisciplinary team meetings.
  • Design community reintegration activities to support physical reconditioning and psychological adjustment.
  • Recommend and/or design adaptive equipment to enhance home or work environments and participation in rehabilitation.
  • Train staff and family members to carry out retraining programs (e.g., dressing, feeding, grooming, hygiene).
  • Supervise, instruct, and evaluate Occupational Therapy Assistants and Home Health Aides in delivering services.
  • Accurately document patient care, interventions, and evaluations on the day services are rendered; ensure timely implementation of verbal physician orders.
  • Submit evaluations, treatment plans, and discharge summaries to supervisors and care management staff.

Requirements

  • Current and unrestricted Occupational Therapist licensure.
  • Current CPR certification.
  • Valid driver’s license, auto insurance, and reliable transportation.

Preferred Qualifications

  • Minimum six months of occupational therapy experience.
  • Home Health experience.

Pay

  • $43.00 – $60.00 per visit/unit.
  • $63,100 – $86,400 per year base pay.
