Responsibilities

  • Assess and screen patients' daily living and work-related skills, then develop therapeutic retraining programs with measurable objectives
  • Administer and interpret diagnostic and prognostic tests of function in patients' homes or other settings to assist physicians in evaluating function levels
  • Confer with physicians and healthcare team members to develop or revise the Plan of Care Treatment
  • Provide therapeutic treatment and instruction to patients per physician orders to improve or restore strength, coordination, range-of-motion, and function, or teach compensation measures
  • Review and expand retraining programs consistent with each patient's behavioral goals and rehabilitation program guidelines
  • Consult with vocational team members to develop and implement vocationally oriented plans aligned with patients' needs and capabilities
  • Report patient status and progress to physicians, rehabilitation staff, Clinical Managers, patients, and family members
  • Participate in care coordination and discharge planning; act as a resource for healthcare personnel
  • Design community reintegration activities to assist clients in physical reconditioning or psychological adjustment, coordinating with interdisciplinary teams
  • Recommend or design special adaptive equipment to improve clients' residence or working environments or participation in rehabilitation programs
  • Train staff and family members to carry out retraining programs, including dressing, feeding, grooming, and hygiene skills; participate in team and family educational meetings
  • Supervise, instruct, and evaluate Occupational Therapy Assistants and Home Health Aides in implementing services
  • Document patients' care observations, interventions, and evaluations on the day services are rendered; ensure accurate documentation and implementation of interim (verbal) physician orders
  • Submit evaluation, treatment plans, and discharge summaries to supervisors and care management staff

Requirements

  • Current and unrestricted Occupational Therapist (OT) licensure
  • Current CPR certification
  • Valid driver's license, auto insurance, and reliable transportation

Preferred Qualifications

  • Minimum six months of occupational therapy experience
  • Home Health experience

Pay

  • Per Visit/Unit Rate: $70.00 - $97.00
  • Annual pay range (full-time, 40 hours per week): $102,500 - $141,000, based on geographic location, skills, knowledge, experience, education, and certifications

Schedule

Scheduled Weekly Hours: 40

Additional Information

  • This role is patient-facing and requires Tuberculosis (TB) screening
  • This role requires a valid state driver's license and maintenance of personal vehicle liability insurance (minimum limits: $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage, or state-mandated minimums, whichever is higher)
