Occupational Therapist, Home Health
About the Role
A Home Health Occupational Therapist assesses and develops therapeutic programs to improve patients' daily living, mobility, and independence within their homes. They collaborate with physicians and healthcare teams to create and modify treatment plans, deliver therapy based on physician orders, and educate patients, families, and staff on rehabilitation techniques. The therapist reviews and expands retraining programs, designs community reintegration activities, and recommends adaptive equipment to enhance patients' environments and participation. They supervise and evaluate therapy assistants and aides, document patient progress accurately, and participate in care coordination and discharge planning.
Qualifications & Skills
- Degree from an accredited Occupational Therapy Program
- Current and unrestricted OT license
- CPR certification
- Good organizational and communication skills
- Reliable transportation, valid driver’s license, and auto insurance
Preferred: at least six months of occupational therapy experience, home health experience
