Join Our Therapy Team at Visalia Post Acute
Visalia Post Acute is seeking a compassionate and skilled Occupational Therapist (OT) to join our dedicated rehabilitation team. This full-time opportunity is ideal for an OT who is passionate about helping residents achieve maximum independence, function, and quality of life in a skilled nursing and rehabilitation setting.
Responsibilities
- Evaluate residents within 24 hours (or one business day) of physician referral; document delays appropriately
- Develop and implement individualized treatment plans and obtain physician approval for services
- Provide occupational therapy treatment per the physician-approved plan of care
- Supervise Certified Occupational Therapy Assistants (COTAs), aides, and students in accordance with the state practice act
- Communicate resident progress, challenges, and plans with the rehab and interdisciplinary teams
- Participate in patient care conferences, utilization review meetings, and rehab meetings as needed
- Complete evaluations, daily treatment notes, weekly progress notes, recertifications, and discharge documentation per regulatory and payer requirements
- Education and training residents, families, and caregivers on safety, adaptive techniques, and home programs
- Participate in discharge planning and coordination of care
- Recommend and facilitate ordering of durable medical equipment (DME) to promote functional independence
- Conduct in-services and training for facility staff as assigned
- Report equipment issues promptly to ensure safe and effective operation
Requirements
- Current, active California Occupational Therapist license in good standing
- Must meet all continuing education requirements per state regulations
Qualifications
- Bachelor’s degree in Occupational Therapy required
- Master’s or Doctorate degree in Occupational Therapy preferred
- Skilled nursing or geriatric experience preferred but not required
Skills
- Proficient in electronic documentation systems (Casamba and PointClickCare preferred)
- Strong clinical judgment, communication, and organizational skills
- Ability to work independently and as part of a collaborative team
Physical Requirements
- Frequent standing, walking, pushing, pulling, and reaching
- Ability to lift and move up to 50 pounds frequently
- Occasional stooping, kneeling, crouching, and balancing
- Visual and auditory acuity sufficient for patient care and documentation
Work Environment
- Skilled nursing and short-term rehabilitation setting
- Noise level typically low to moderate
- One primary work location
