Responsibilities

  • Participates on committees as requested by the supervisor.
  • Delivers the highest level of service to all residents, families and visitors.
  • Models and maintains a safe environment in which to deliver services and employs correct body mechanics during activities.
  • Demonstrates and implements appropriate treatment techniques and skills with residents.
  • Completes resident treatments as scheduled.
  • Gathers all information necessary to properly evaluate the patient's needs as per department policy, including testing and consultations with other health care professionals, as appropriate.
  • Discharge planning established at initial evaluation and updated through the course of therapeutic intervention. Discharge summaries are completed in a timely manner, reflecting patients' functional status, long-term goal achievement and appropriate recommendations.
  • Demonstrates appropriate knowledge in assessing the need for DME for residents to improve their functional independence safety. Follows through with appropriate professionals to ensure proper fitting of the wheelchair to the resident.
  • Prepares written documentation such as Progress Notes, Reassessments and Discharge Summaries. Progress notes and Reassessments are completed as per departmental policy and procedure, and monitor the resident's status and progress towards goal achievements. Re-evaluations are completed in a timely manner. Goals and treatment plans are updated as per department policy.
  • Develops a rapport and mutual respect with the resident to foster an optimum environment for goal achievement. Provides education to residents/families based upon the identified learning needs, in a manner that the residents/families can demonstrate their concerns.
  • Recommends and provides education related to utilization of appropriate occupational therapy equipment (i.e.: reachers, splints, etc).
  • Develops and implements an individualized program of treatment focused on identified resident needs. Focuses on achievement of the established goals. Participates in the ongoing development of the discharge plan that was initiated at the time of initial assessment.

Qualifications

  • Bachelor's Degree from an accredited school.
  • Demonstrates the ability to learn quickly and adapt to changing patient needs.
  • Strong sense of accountability for improving the lives of patients and their caregivers.
  • An exceptional focus on teamwork and the ability and passion to deliver the highest quality of care based on a strong sense of patient focus.
  • Excellent written and verbal communication skills.
  • Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.
